What to Expect at Your First Appointment:
We ask that you come 15 minutes early for your appointment in order to complete paperwork and the new patient check-in process. Whenever possible, we mail out new patient packets which we encourage you to bring completed to your first appointment. You are also able to download these forms from this website if it is more convenient for you. Once you have an appointment scheduled, we encourage you to request a My Chart Patient Portal account. It is an excellent way to keep in touch with our office and be more involved with your healthcare.
